You need to translate \scripts\definitions\presets.rpy with a code like this:

Where I put es_es put it_it or whatever you called the translation. And change the Spanish thing to Italian. You can use this file to translate more things like:
- Khensbish recipes
- hinterland journal
- contract names
- memento names
- file display names
- reward names
- reward display name
- item descriptions
- guest descriptions
- stat descriptions
- And some clothes names too (i had some trouble with clothes... it's like some translation in this file and some in other file)
You can download the spanish translation to check those files and modify them to Italian
