This game is a genuine masterpiece. 

I've already played a ton of R18 Games, but this easily stands out among them all. And it's not just the sexy times by a long run. I relly got invested in the story all the way through, the premise was very engaging and while the characters of course filled classic roles, none of them seemed bland or uninteresting. I was truly happy to see the perfect ending I managed to achieve. 
I also have to mention the humor of this game. I've rarely seen a game pull off silent humor as well as Once in a Lifetime. There were too many times where I laughed out loud than I could list here, but basically, everything involving a certain character of the stabby variety was hilarious and made me fall in love with this game over and over again. 
But at the same time, when the game wanted to be epic or tense it delivered wholeheartedly, the town and the asylum genuinely gave me the creeps and had me tense the whole scene through. 
I could probably go on forever just listing off different things I loved about this game, but the best way to experience it is to just experience it for yourself and I can say I am glad that I bought this game on a whim.
