Weight gain, lots of it. Rare to see it done in a full-3D game, even if the character models don't look that great.
Annoyingly, one character is real big into water distension enemas but you're forced to turn her into a feedee and make her obese, followed by her never doing water inflation again.

The gameplay's pretty jank, there's bugs and all that, and the most annoying part is that mouse sensitivity is about 10x higher than any other game and you can't even change it.

Anyway, what saves the game is that the character models are able to expand so much. I doubt I'm that far into the game (I quit now, BTW), but I'm already seeing plenty of pretty dang big bellies.

Probably should rate this lower, but ehhhh.
