I've encountered this bug where some items randomly duplicates itself but they're linked so if you use or sell one of them, both will decrease so it's more of an illusion, only issue is that it still takes up inventory slot.
The same. I have two packs of 57 poison powder each, so it should be 114 overall. But when I come to improve the skill for applying poison to weapons the dialogs says I don't have enough., which is a 100. I can see flowers and mushrooms packs repeat twice also.
