I know its a just a small improvement and not really that important but it would be nice to be able to receive notifications when on the site at the time of the notification rather that the next time I refresh the page. 
I'd also be interested in a browser extension that could let me receive my  notifications without having to go to itch.

I think this would increase engagement between people, because sometimes I don't know for 5-10 mins that someone has left me a comment or asked me a question and being able to reply while they are still online would allow for more conversations

Itch as a whole is not designed to be a real time social media. People are not "online" on itch to chat. And they can activate email notification for project updates.
