Not sure how the story worked with side content, but does doing the underworld change anything for the main story? If possible at some point I would like to know the endings that are possible to see what I got.

Anther thing i never tried, how picky can you be in the game with the girls?

You may get some minor dialogue variations by going to Underworld first. But nothing major. It's mostly the talking to ghosts stuff.

I'd rather not post spoilers about endings here as Itch doesn't have a spoiler tag. As for being picky, I'm not sure what you mean? There are some side characters you can simply skip.

Some dialogue made me think maybe I can even let'S say focu in the elve? (can't write her name properly yet Alryth?)

Feels like I'm thinking bigger than what the options currently are.

Changing the heroines, right. You can use it to change characters in the gallery and glossary. But that's about it. Anything more would require a lot of extra work.
