In 8 Bits to Infinity jams of the past, there was a form of questions that popped up when I went to submit my project. This time around, there seemed to be only one question. It asked which Track my project pertained to but that was it. Was there supposed to be more? I expected it to prompt me to list the changes I made to my initial build but no. Is it this important that I outline the changes on my game page?

Hey Magoo, that is indeed the complete list of questions. It would definitely be cool to see a changelog of sorts but it's not required.
