I feel too much of this One Page RPG is taken up with character generation and not enough is given over to the main threat of the game and making this threat different and interesting. That's the only major flaw of this. Perhaps this weakness is because the author is trying too closely to mimic the VHS horrors  mentioned in the text.

My tired brain is just not following what’s going on with the bottom half of the sheet +2D +1D +2RD what are these mechanics?  I suspect I'm just being dumb here.

I think it needed to build a proper horror crucible – where you have the reason why you can’t stay (obviously the supernatural enemy), but you also need a reason why you can’t leave the setting, the dilemma between the two threats would add considerable tension.

The levitation girl drawing is very spooky, and the plot twist table is good idea, as the game's title, but I think any GM wanting to run this is going to have to work to prepare some ideas for this before he even thinks of bringing it to the table.
