A deep and compelling gameplay experience with lots of content.  The time spent tweaking and honing the systems really shows.  

When it comes to purely inflation, weight gain, and pregnancy content the size and variety of content is perhaps slightly more limited than games specifically dedicated to those, but the integration with the world and general interactivity more than makes up for it.
