Most likely someone who is unfollowing then following again. Do you mind sharing any of these accounts, I’m curious if they are doing this to try to spam creators. In which case, duplicate notifications should not be allowed.
I can't say for absolute certain because previous follow notifications seem to removed once a new one appears, and I don't want to accuse anyone without proof. But I'll start keeping track just in case this does become an issue
