Maybe I just don't know where to look for this info, but I can't figure it out. How are these expansions added on to the game? Could I add them while in the middle of a playthrough? Can I (should I..?) play with several expansions at once, or should I limit it to one?

There's no rules for how to add them. You can indeed add them during a playthrough and I would recommend adding them whenever you want to introduce new content.

Nightfall is probably the best expansion to get first as it doesn't introduce new plots, just night-time foraging and a few ailments.
