How do I complete the hot spring quest? It’s been probably a dozen days and no matter how much or how little I bathe or how much I meditate or don’t meditate or if I eat or not and the bartender is no help he just tells me to bathe at the hot springs.

The amount of time you spend meditating is what matters. Don't spend too much time in the water.

If you do everything as Kimiko says, the effect takes effect within four in-game days. Tested in version 0.74. On the fourth day, Nel arrives and does everything as expected.
