I love it but I don't know how to not get my intestines out or kill myself and I tried to be nice but I don't know how to literally get past it and now I finally got past where I died apparently the heart thing is the only thing that keeps you from death or love
The heart meter is a visual indicator of Akano's like for you. Most endings will trigger by the heart meter being at a certain fullness at different points of the game which is based on your choices. So in a way, yes the heart is the only thing keeping you from death, but it is entirely determined by the many different choices you make.
