that’s a good idea, didn’t even cross my mind! are you thinking of a form for applying, or for submitting finished work?
one for applying might be best so folks can email you the final pieces rather than put them in a form. but, that is ultimately up to you! You could also do a form for just submitting if you aren't worrying about vetting entries
