Is there a way to see: when titles upgrade/how to get them/how many there are and what the max level of the titles are?

You can see the full title list in the mirror (at the top of the default list, there's a 'show all titles' button, which lets you cycle through all of the titles and their level descriptions). The most recent premium version (which will be free very soon, as a new version is almost ready for release), also has a titles preview screen before death to let you see what titles will be earned in your current life.
