Hmmm, there can be some bugs if you’re using an older save from a few patches ago, but I’ll try to help figure out what’s happening.
I assume you mean Mara’s scenes in the gallery? Her second scene (the blowjob) comes from the quest where you get her a bed. If you’ve completed that quest, the scene should be unlocked - if not, it might be because you’re using an older save file. You can unlock it on any save and it will count for your main save, so you could do a quick run to the side quest (right after you meet her) to unlock it for all your saves.
Isabella’s side quest unlocks after you reach over 10 affection with her. After that, you can ask her about why she looks tired. If that option isn’t showing up for you, it’s almost certainly due to an old save file where some variables didn’t update correctly - they should normally change during a standard playthrough.
We're sorry about that - as we continue updating and adding to existing content, unexpected issues with old saves can sometimes pop up. We always try to keep at least the last three patches compatible, but it really depends on where you left off in your playthrough.
