Can I check then - maybe I'm misunderstanding the difference between "Fixed Levels" being on and off? I assumed that fixed levels was a standard set of crafted levels and turning that off randomised those levels. Does that mean they're effectively procedurally generated but "Fixed" uses a set seed to create them?

All the levels are generated procedurally. The fixed ones, however, use - as you correctly guessed - a fixed seed, so that they repeat always identically (that is, other than for the background colors, which always change). That is necessary to makes scores comparable. (Side note: when the FIXED LEVELS option is off, the scores never make it to the hiscores chart.)
