If you get one of the girls pregnant in the dungeon, then leave without freeing them and re-enter the dungeon to free them, they will become fully pregnant between those two scenes. This is pretty consistent, but not 100%.
Ah, yeah, the coding doesn't really account for not freeing them. To be honest, I don't think this will change.
