It sounds like you built homing orbs that flew towards an off screen enemy once placed. The mushroom stew probably went to a different part of your inventory once crafted, once a slot is cleared in the inventory the newly crafted item takes it instantly. I assume when you crafted the stew you ran out of mushrooms and/or water so that cleared up a slot or two and sent it to a part of your inventory you didn't expect it to go.
