Okay, I think getting an "emerine crystal" will start the heart process. You're just gonna need at least one and talk to him about it. If you want to make it faster, just collect at least 5 to 10 emerine crystals instead of going back and forth to the cave.
This is from what I read about in the wiki. But now I have this problem whether I want to restart all over again or not because I didn't get a relationship with Gunnar. What's holding me back from doing so is the Bareshade expansion and the junkyard event... I just hate the rats.
i've already done all of it,
i have like 20 crystals prepared for him it's not enough like the game said don't be too "eager" tbh i try everthing like
- walk around and protect him and give him 5 crystal not working
-eat him out 1 times protect and crystals not working
-eat him out 2 times protect and crystals still not working

Did you get the scene on the way to the mine where he has you give him oral, i didnt get that scene until i had gotten then northcrest guards reputation to a certain point, Dulrag will make a comment how popular you are... after that just ignore him putting his hand then im not sure if the second guard interaction affects his heart i went with it, and in the cave protect him, eat him out either once only or twice only not entirely sure (i did twice) and then give him the max amount of crystals you can. That should give you the heart.
deleting cause I just saw you figured it out lol. but wanted to mention, Feminine juice only makes you smell like a woman, it doesn't change your gender. there isn't anything in game that can do that right now lol.
(also the only thing that relies on smell to find you from what I remember is the tar things near Viccoria.
