I was trying to get this item in my latest playthrough, found it in the dream, but it didn't trigger the dungeon to get it. Has the item been removed, or has something else changed so it's harder to get now?

Edit: Nevermind, I forgot I needed to talk to Myr to activate the quest to get it.
