On Windows/Linux, you can just extract it anywhere and run the Mannequin executable to run.

On Mac, unfortunately the only way (at least for now) is installing via itch.io app which can be downloaded here: https://itch.io/app

The itch.io app installation method is the preferred one, because updates also delivered through it (think of it like the Steam app or iTunes/Google Play Store/Windows Store).
