If you get a task for a girl from a different town, you can use the 'Viewing Sector' dropdown to switch to that town's sector, without needing to take the train back to that old town. The intention is that there's one facility that all the girls from all towns are sent to, so you're already there.
But you definitely raised a valid point, as it turned out that taking the train to a different town dropped your current task. I've released a patch that fixes this (version 5.16).

Your online shop idea would be fun. I'd definitely like to add new cooking options as time goes on, that link in with the various new systems.
I hear you about the fullness system. That logic stems from the original version of the game, which had no days of the week and limited you to one date per day per girl. I'd like to change it too at some point, but it may come at the same time as a wider overhaul of the dating system!
I'm currently working on a mini update that'll let you go through the blob milestones at home. One consideration is the growing space they take up, and adding new buildings to your home could be a good way to tackle this.
The doctor's office should appear as a date location & if you go there with a girl, the focus will be on her. Once you have a prescription for her, you should be able to buy medication for her at the pharmacy. Let me know if that's not appearing for you.
Good idea about asking girls to give up their jobs, making them more available at home.
I hadn't thought about making the girl's home more like the player's. That's something I may add in a future update!
I'll definitely be improving sleeping with girls in the mini update I'm working on currently. I want you to be able to sleep with your chosen girl all the way to the highest blob milestones.
I'll be adding a pregnancy system in the next big update, so I could possibly look at a menstrual cycle in that. But no promises!
I would like to add a university to the explorable London location that'll let you study for a degree, but that won't be coming for a while.
Right now, those jobs are the benefit of picking the graduate background, but you should still be able to find jobs from every career type that don't require a degree. I'll be making some improvements to job switching in a mini update I'm currently working on, which will help.
You're completely right that blob content at home is currently lacklustre. I know it was one of the main selling points of the Jobs & Home update, and likely why it was picked in the poll… but I probably spent far too much time on the portraits and OnlyFats instead!
Before I start work on the Sex update, I want to release a mini update that'll address this. Your ideas are great, and I'd love to add a bunch of new home upgrades targeting characters at the immobile+ stages, especially blobs. You'll be able to progress through the full blob milestones, and hopefully it'll be fun to do so.
I really like how the livestreaming turned out, so you'll definitely be seeing more of that soon, including for blobs.
Health issues will also be expanded in the mini update. Home doctor visits are a great idea. I'll include that.
I want home upgrades to be an evergreen feature, with new upgrades being added over time. It makes sense to have some sex-related upgrades, and I'd be happy to add BDSM equipment.
Someone else suggested linking binge-eating OF videos to the existing binge-eating minigames, with performance affecting the video's success. I'd love to do that for the sex videos too, so expect to see the new sex mechanics used there.

I'll be making a bunch of improvements to stomach capacity in the mini update that's coming soon. I want it to scale with weight rather than have fixed tiers at weight stages. So that'll let it increase past 180 for you.
I'll also be adding a fourth status effect type you can add to snacks/meals you cook that'll temporarily increase capacity.
60% is the cap for new towns (so that they're not too easy to obesify). But the country's rate should continue to increase beyond 60%. At 20 towns, you should be pretty close to obesifying the country. Check the news section for the country's current obesity rate.
When you reach 99%, you unlock a final set of newspaper stories & that's the current end of the road. But I plan to add much more to this in a future update (involving an organisation trying to fight back against Dizzy's that you can help or hinder).
Sorry about that! I've gone through it and figured out what happened. This should hopefully be fixed for good now.
Care assistants used to be much simpler than the normal girls, but they are now fully featured from the start of the game with the most recent big update. There were some old references to how they used to be still in the game.
Right now, you can't delete their profiles, but I'll add that.
I've set a video limit of 50 per model. The oldest videos will be deleted when new ones are recorded at that point.
I've just released update 5.14, which replaces 'All Time' with the previous year, as you've suggested. This graph should display correctly now.

That's right, you'll be able to cook meals for yourself in the near future.
Currently, if a girl eats a snack with the appetite stimulant ingredient, her appetite becomes 'ravenous' for the rest of the day. This provides a bonus to your feeding persuasion chances, amongst other effects.
Fullness capacity is a great idea for a new status effect though. I may add this to an existing ingredient or create a new ingredient with this effect.
The Wild Truffle ingredient also adds a very powerful status effect to your food!
I'll add a new ingredient item soon to the Dizzy's Employee shop that adds lots of calories plus the Dizzy's addiction status effect to your culinary creations.
I also really like the idea of creating custom shakes, I'll add that to my to-do list.
Although I won't include death in the game, I'll be expanding the health system in a mini-update that I'll release before the next big one.
This could work like you describe, but the threat would be girls being hospitalised rather than dying. Maybe the symptoms could worsen and become harder to manage the heavier the girls get.
I've tested these, & they were working, but the amounts might have been too small to really be noticeable. Previously, a mini fridge would provide 200 to 1500 calories per day, based on the character's appetite. The vibration plate would burn 800 calories per day. I've now buffed these to 1000 to 2500 calories from the fridge and 2000 burned by the plate.
I'd love to give that option, but it's trickier than it looks!
For example, what if your player character is trapped in the Dizzy's facility (which is only possible if player weight gain is enabled), and then you disable player weight gain in the settings? Or if you're dating a girl with the Macromastia body type, and then you disable that type from your game.
