I'm very sorry!! I don't know what could be causing the issue, I just tested it with all of the answers in the "Good Ending" section and got the good ending.

If you told Lady Selyse that "He's a Lost Cause" instead of "I'll try my best" it takes a heavy point hit to the bad ending, it's the only thing I can think of? 

I would suggest maybe picking "Freedom" or "Duty" and following that guide along with the Good Ending guide too? I'm really sorry, I'm not sure what to say as I can't recreate the error on my end.
