I think the box you're talking about is the one with the 2 star switches next to it. The floor under the crate itself is destroyable with a bomb.

The difficulty settings don't do much outside of boss fights, ideally I would remove enemies in lower difficulties. And yes, some of the rooms are a bit of a mess. Many are marked to be re-tuned, some redone entirely. Unfortunately I doubt I can ever get back to this project. Thanks for the feedback, though!
