So I just found how it work. Somehow the game have a system which new generated npc will have level equal or more than player even if the level up system off. 

I run some test when I reset my level to 0 the new npc is also 0 but if I have level increased maybe 1000 they also spawn with that amount of level.

And this especially apply for encounter npc in danger zone they will always equal or more level than player. 

1. What about new generated npc set level? Idk how but after I disable that when I encounter new npc and fight their level  scale equal my  um cheat character when newly generated/encounter

Go to medical floor on the bottom right side there a computer icon. walk there choose nurse and choose all option to unlock children content.

1.How to disable npc level scale to player??? or they level up after fight with player

2.How many main npcs quests are there in game?
