If you kill any enemy in the game for a few moments befor they hit the ground an item is in the body. Easiest to re create this with the big trolls. Kill them fast and hit the accept button as soon as the prompt shows up. If you grab the item befor the body hits the ground you are stuck looking at the item you picked up. No option to accept or continue.. Good job, now all you can do is force quit the program.
