Literally in the second location there is an entrance to a cave with a lot of flydick in it.

Loader version is full but it can take a long time to download from the servers. Other version is separate:

If you want to play as a female you download the female version. If you want to play as futa you download the futa version. If male you download the male version.

How and where do you run the game?

It's a compressed zip file. Unarchive it with any program such as 7zip or Winrar

You don't need to do any quest more than once to increase HR...

Have you tried doing the quests at Gaffer Choo's? If not, you should try it, it's the only way to increase HR and you don't have to be a patreon member.
