He doesn't appear in the orc camp of the battle grounds near where the city is. He appears after the orc war(siding with Korg).

The orc camp you need to be in is much more above the map, you'll see a cave directing up there. (It's a long journey to continue this). 

Once you managed to finish some tests given up there, you'll be able to walk around and enter the battlegrounds there, he'll appear with a dialogue wrestling over other orcs in that camp.

The hot spring is at bareshade around the left area side. You'll see a guy wearing a white robe there telling you the private and public baths.

Honestly, this game is the best I've read so far from the other nsfw games I've played. It made me appreciate the lore, openess of the character who to date and the bara art style. I thought it would be my usual reads but it's the one that keeps me up for the weeks to finish. I wish the orcs have more updates. 

Either win or lose in the fight at Wednesday of the tournament. Then in the feast, you pick Maul to be with you and then a couple options later, you'll encounter the it.

You will have to go the the event where Gunner and the merchant is in the middle of the road of bareshade and your home. Throughout the route, they will give you a horse and a new location you will meet them in far above the map.

You can open the cheat codes there on the preferences and access potions to threw it off distracted. For example, using a sleep potion at her and then escape if you encounter it again. 

Or if you wanna go hardcore, stack the strength potions and hit it and get its hair.

At first in the game it's vanilla. But once you continue on the game and went through routes and fighting off monsters, there's plenty of fetishes.
