1. Yep, that only starts with the Ace of Hearts. Some playthroughs go roughly. This is a highly lethal game. 

2. Only if you draw the aces and 8s on the same hand. Otherwise it would basically be inevitable.

The answer to both questions is in the same place: page 14, under the  Ace of Hearts. I will retype them here for your benefit:

"Do not discard this card. Instead, set it to one side in plain view and place ten tokens on it. At the end of each day, roll a d6. On a 6, remove one token and consult LAYING A GHOST TO REST (p. 23)."

HARD MODE: Treat it like confirmation of the rumor... with crucial details contradicting the previous rumor. Add both.

Treat it like secondary confirmation of the rumor - add Motive but no Doubt.

this is the only one I'm aware of, but they appear to be sold out. I'd ask the shop about stocking more!

Just all at once on one draw, not over the course of the game. It's basically a surprise instant death but exceedingly hard to get.

The Motive value just signals when you've compiled enough evidence (or "evidence") to finally point the finger at a Guest. In the fiction, it's when you've got enough to potentially put someone away. In the numbers, it just guarantees play will go on long enough to introduce complications.

Correct, every time after the FIRST draw. Second, third, fourth, etc all add Doubt to anyone attached to that Motive card. So, for example:

1. Draw a Motive against a Cultist, Cultist gains +1 Motive

2. Draw the same Motive against the Medium, Medium gains +1 Motive, BOTH gain +1 Doubt

And  "KA" is a police acronym for "Known Associates." If you want to keep tabs on who is consorting with who.
