MAJOR SPOILERS FOR ONCE IN A LIFETIME
In OIALT, there's also a pregnancy question at the start of the game. If you do enable it, some girls will get pregnant if you creampie them, though this is late in the game. During the main story, pregnancy is mostly limited to the characters finding out about it, we see only very early pregnancy. It's an additional, wholesome part of the story! Though there are no additional lewd scenes. If you get the best ending, there will be a collage of what happens during the year after the finale. If you enabled pregnancy, a few of the pictures show that all girls in our harem eventually get pregnant and we even see some of the babies.
