The art is very cool and the atmosphere is a bit dark and I like it, the animation to switch the form seems super good.
On the downside the camera have the controls inverted or something like that and can be extremelly difficult to control properly. Mouse sensitivity was another issue, but as you tried to make an option to adjust that I guess you didn't had the time to implement the option properly.
