Items with a (? ? ?) are ones you can unlock by completing certain actions, although some are unavailable in the demo. If an option is crossed out, it's Patreon-specific. If it's in red, it's not implemented in either version yet.
Sorry to hear about the bad conversion rates. The game will sell for pretty cheap once it's done, so hopefully that'll make it less painful for people in regions with worse conversion rates to USD :(
