It is the room with the dragon stone with the torch on either side of it. Any time I try to go up the stairs with the daughter from the cellar to right in front of the stone in that room, the screen goes to black and I get an automatic game over via the hypno bad ending.
I also tested getting a different sex scene via the normal guards, that automaically goes into the hypno bad ending instead of the usual scene too.
