The 3rd ending is if you previously did not tell Ted you have a crush on him, and if you said no to having sex (if you're playing the nsfw version). Though you still have to spend time with him at least once to get on Ted's route in the first place.
From there, if you choose to leave at the end, you get a different ending that's similar to the first one, though it plays out a little differently.
