Hmmm, could you load a save on the new game after the scene is supposed to happen and open the console (Shift+O) and type jade_submissive? If that value returns True I think I need to investigate further.
When are you checking the variable, it will be False in the main menu and throughout the game until Episode 12a? The jade_submissive variable is set in Episode 11. So if you were strict with her in Episode 3, 4, 5 and 7 the variable should be set to True in Episode 12a.
Could you check the output of ep002_jade_sex, ep004_jade_sex and ep009_jade_sex_accept by typing them into the developer console. They should all return True.
