Thank you for posting :) This game has a lot of potential for soft locking. Right now the only way to deal with it is to hard reset with "ESCAPE" button. About mentioned ones:
1. known one. It can be fixed by expanding world to be less linear.
2. first time see (orb room) :D good one :D
3. in all rooms with enemies, there should be a way to go back and bring what you need. Lesson learned :)

I have a few solutions for next version for now:
1. take it as a part of gameplay and make it less painful
2. track and eliminate all posibilities of soft locks
3. apply rollback mechanics (undo/redo)

For number 3, you you bring just yourself, you can escape, but putting the sword behind you makes it impossible.
