There is a new action for the over the knee position. Only works with the headmaster and not Sally. Also make sure you've completed everything in her quest log.
My quest log says "Teach a class and make sure Rachel stays awake". I tried all the options available but it doesn't seem to advance the quest. I'll go tinker a bit more.
