Well I'm pretty sure the bug starts to trigger after miles 3. In miles 3 you weren't supposed to light your parents with the flashlight because it can result in a car crash, but here the bug causes you to lose even though you didn't point the flashlight to your parents. After that, almost every time you use the flashlight, even on miles 1, you have the probability to lose instantly. That's all I know. If it still doesn't explain it, you can check my let's play video. In the middle of the video I've to deal with that bug and somehow manage to advance through the game.
