Feeders need to have unlocked the Dizzy's Delightful Doughnuts feeder upgrade before they'll take over the shop, maybe that's the blocker? They also need to reside in that particular town and not be immobile.
I'll make sure it's possible to give weight-loss medicine to feeders in the next update. There'll also be a bariatric surgery option for faster, but more expensive, weight loss.
Please DO NOT add surgery. That sounds horrific and destroys the vibe. I would prefer to avoid this horror game feature.
Different medicine types should be enough (dozens of pill and injection options exist). But if you really want to add surgery, then there must be some serious warning signs also included like "permanently disable gaining ability for that citizen and cannot be undone", which show that there is no way back, and also terminates the model career as followers would not like it.
Instead you can add some real life health maintenance options which are very common among heavy people like high blood preassure (can be treated with pills), and diabetes (can be treated with insulin). These don't have weight loss effect, but would help to keep up good health conditions and good mood even at USSBBW stage. (These could be daily routine and/or doctor's visit gameplay options)
There's just one thing about this game. Oh yeah, it's not real life. In real life it wouldn't make sense to gain like 5-10 pounds from eating 20,000 cal cookies. I for one SUPPORT this feature because there are no quick ways to lose weight at higher weights. Maybe a few additional methods could be added, but I'm not in favor of getting rid of it.
I think I understand your fear. BUT, I don't think there would be an extremely in-depth description to the point of blood or such things, but there will just be enough stating of what is going on, just like when you are in the dizzy facility as a blob stage character (maybe). So I think you can rest easy and trust her on this addition.
I have a couple of the feeders at or above level 18, but it doesn't seem like they are taking it over in favor of their current occupation. It's not the biggest deal, as I just wanted to try this feature. I did get it to work on a newer save, though. And the surgery honestly sounds great. Trying to get through like 50 weight-loss doses takes forever, even with the XP boost
