Hello! Have you installed both monogram-extended.ttf and monogram-extended-italic.ttf? Each should contain the regular and italic subfamilies, respectively, and your OS should be able to handle the styling contextually, when selecting monogram as the typeface.
I don't have a macOS device on hand to test your specific case, but this setup works fine on my Windows 11 machine.
