Prerequisites from previous version - Meet Harleen in Asylum. Recruit Wendy as truck assistant and complete Thundergirl's quest.
1.Enter apartments map. See if lights in 201 is off(if you are playing on easy or normal of course) if it's off. Enter the room and find a portal at the end.
2.In other world, talk to Katherine, get wine from her, talk to Masumi until she say she is feeling dizzy.
3.Go out and enter apartment 202 in other world. Find Masumi there, try having sex with her to get other guy mad at you. Keep it up until he hits you. After that Regina should tell you to do the nurse quest.(It's bugged till this point. You can reset recruits questline and try again or wait for next update.)
4.Do asylum mannequin puzzle. Go back to Regina in other world. Talk to her a bit. She will ask you to give her some privacy. Go in normal world for 10-20 seconds. Come back and you should see Rachel. If you have Mrs. Upton in apartment 201 you can get her cloths for Rachel which are outside of the apartment. Second cloths are given by a lady in the asylum. For third one, do a Maya's quest in her apartment.
