I am so confused lol. Where in the game did it reference Lera being trans??

Never said it explicitly did. Just that you can interpret various parts of the game as being about the experience of being a trans woman. Things like Lera being repulsed by her own reflection, or other children her age calling her gross and weird, or the way she styles herself. Even broader things like her listlessness and depression at the state of the world, the way she walks through life like a corpse, it's something a lot of trans people can relate to. If you don't interpret it that way then that's fine, different people will get different things out of media.
