The sounds and voice needs improvement. Currently it sounds quite mundane. The stream, its taking me too long to get into platinum, maxed relationship, 100k+ creds, 10 min vid, all things purchased yet still at gold 3. Might be skill issue on my part. Would appreciate it if you plan to update the X ray visual, shot 10 time yet still not full lmao

Sounds were added in 0.1 and have never been updated since then, so yeah...
As for the stream part, the grind is heavy and I dont think its skill issue since you're hindered by the number of requests you get and to raisn the access level you need to fulfill them - it's not confirmed but I have a hunch that dev deliberately made it that long as a preparation for future updates or new items locked behind certain access levels (such as upcoming condoms will be available if you reach silver tier)
The x-ray - it has already been updated, though it's a simple animation that plays whenever mc cums inside of her, the game isn't ready for multiple creampies yet, but there are plans for that feature to be added eventually
