For the interaction you've outlined here, the intent is that you'd lose one health but ONLY ONE health in this scenario as you'd not have the required food. That said, if any situation like this ever comes up and the rules don't specify, you always have blanket permission to do whatever makes sense and is most fun for you. I never want people to feel "stuck" while playing.
