The art is done with AI, but is heavily edited and modified afterwards, some things are fully drawn like some of the girl's hair like Charlotte and Amelia. That's what's done with Ai.

If you mean operation as surgery, it was to do with her breasts but is left open to interpretation what kind of operation.

I've seen many people complaining about the us of ia in games, but yours is a great example of how it should be use, as a tool to suport the artist instead of just let is do all the work

And since anna say she is all natural is sure it was not stetic
