Within the flooded caves dungeon, a specific tile set piece (a sort of pseudo-spiral that's only 1 wide) that spawns often has a chest at the end of a little passageway. For some reason that specific chest spawn counts as either unable to be accessed or owned by someone else, and you can't loot it when it spawns.
