Fantastic prototype. I will follow this with interest.
I see three main strong points in your concept :
1) encounters (do flesh them out to the level of detail of the classics of the genre and beyond!). The detail in the oral sex was excellent and left me wanting MORE.
2) survival (you nailed it, the city is brutal)
3) transhumanism (very strong base system you got there).
If you continue in that direction, make that city more dynamic and alive, build a solid interface and maybe some visuals, then you have your place for the GOAT of SugarCube games.
