What many NSFW or falsely accused sfw games do, is to remove the optional donation payment to itchio. Afterwards it gets reindexed in like a hour at most.
(Edit: You can see such examples with Polar Night or Remember the Flowers, which are not really +18, but have tags or being marked as adult. So they had to remove the donation option to not be de-indexed.)
