I haven't played the game in a while but I believe you can create a custom gender where you get to choose their genitalia, pronouns, and orientation.
For example: you can create a cuntboy boy choosing a flat chest, a vagina, and male pronouns if you choose that. I'm not exactly sure if it's still like this but I hope this helps.
